*Course Fee Includes: Three Self-Study Web-Based Modules followed by a One-Day Interactive Program held at ZimVie Institute, one round-trip airfare ticket up to $400 (including $35 agency service fee) from the clinician’s home city to Palm Beach Gardens, FL, two nights lodging at the host hotel, transportation to and from the hotel and ZimVie Institute, plus continental breakfast, lunch and social.
Travel to a ZimVie Institute course is for U.S. and Canada based clinicians to Palm Beach Gardens, FL, or Carlsbad, CA. ZimVie Dental will make lodging reservations once registration is confirmed. Air travel must be booked through the ZimVie Dental travel agent within two weeks of registration confirmation. Travel insurance and other insurance are not provided by ZimVie Dental. Registrant is solely responsible for obtaining all insurance. Detailed instructions on booking travel will be sent after registration is confirmed.
